Any information I provide will be accurate to the best of my knowledge.

The recommendations I will receive from The Pharmacist Nutritionist are not a substitute for medical advice from a qualified doctor.

The advice I will receive from The Pharmacist Nutritionist is personal and applies to me only. This advice may be ineffective or even harmful when applied to others.

I must inform The Pharmacist Nutritionist of any changes to my health or medication for the duration of my nutrition plan.

I must inform The Pharmacist Nutritionist promptly if any of my new changes in diet or lifestyle start to cause me adverse effects.

I understand that any changes in my diet and lifestyle may produce gradual—not instantaneous — effects on my body, energy, health, and condition.

I understand that although The Pharmacist Nutritionist will endeavour to help me achieve my nutrition and lifestyle goals to the extent possible, I may not fully attain my goals due to factors outside the control of The Pharmacist Nutritionist.

I consent for The Pharmacist Nutritionist to contact my GP/Seek advice from a private GP (if your appointments are within a private GP clinic) if there are any concerns over my health.

For patients with underlying health conditions: I consent for The Pharmacist Nutritionist to share information regarding my plan, progress and monitoring requirements with my GP
- Payment for each service will be made at least 7 days prior to your consultation, unless a short notice booking is made, in which case payment will be made on the same day as the booking is confirmed.
- A full refund will be given for appointments cancelled more than 7 days before the appointment date. Appointments cancelled with 7 days or less notice will be refunded minus a £45 fee to cover the pre-work done by The Pharmacist Nutritionist. Appointments can, however, be rescheduled free of charge up to 24 hours before the scheduled appointment.
